The primary purpose of this job position is to assist in planning, implementation, and evaluation of the Activity Program, which includes recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident's assessment and care plan and as may be directed by the supervisors.
QUALIFICATIONS:
l. Must possess a high school diploma or equivalent
2. Experience not required but preferred. On the job, training will be provided.
1. Assist in planning, developing, organizing, implementing, and evaluating the activity programs of this facility.
2. Observe resident attendance, mood, behavior, and degree of involvement so that facility activities and resident progress are evaluated by noting, reporting, and charting resident behavior.
3. Participate in discharge planning, development and implementation of activity care plans and resident assessments.
4. Interview residents or family members to obtain activity information.
5. Assist in arranging transportation to other facilities when necessary.
6. Refer resident/families to appropriate social service personnel when the facility does not provide the services or needs of the resident.
Physical Requirements:
Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, and exposure to temperature and humidity. Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet. It may be necessary to assist in the evacuation of residents during emergency situations.
Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
Mental Requirements:
Must be able to work independently and make judgments based on assessments and data available, and act accordingly. Must be flexible, innovative, and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Working Conditions:
Be able to tolerate exposure to elements, including but not limited to odors, blood, body fluids, and excrements, adverse environmental conditions, and hazardous materials.
Salary: $21.00 - $23.00
