We are currently looking for a Continuous Quality Improvement (CQI) Case Manager to join our residential team. The Case Manager will be responsible for ensuring each client receives core services and appropriate additional treatment and services as indicated through comprehensive assessment, and in alignment with their individual needs and services plans. The Case Manager will also ensure that required documentation is current and complete at the campus, including the intake study, the needs and service plans, and transfer and aftercare plans. Monitor client files for progress towards goals, collaborate with treatment staff and child and family teams to ensure core services and other requirements are met. Gather and create reports to include outcome measures for various traumas such as depression, anxiety, and living skills.
Skills/ Requirements: Education: A Master’s Degree in Social Work or Social Welfare, Marriage, Family and Child Counseling, Child Psychology, Child Development, Counseling Psychology or Social Psychology from an accredited school is required. Demonstrated ability and leadership through a minimum of one year related experience in the field of child or family welfare services or as a Social Worker required. Must be at least 21 years of age or older, able to pass a physical examination; drug clearance; criminal background check; Child Abuse Index check. Must hold a valid California driver’s license with clean DMV history, be able to safely operate a motor vehicle and be insurable by Trinity’s insurance carrier. Must have a special interest in working with youth and must fulfill the general requirements as established in the California Administrative Code, Title 22. Be able to communicate clearly and concisely, both orally and in writing in English. Bilingual in Spanish is a plus and strongly preferred. Compensation commensurate with experience.
